Prevalencia de fragilidad en población mayor desde una perspectiva de determinantes sociales de salud

Objetivo: Estimar la prevalencia de fragilidad en población mayor residente en España y analizar la influencia de los determinantes sociales de salud (DSS) en la prevalencia de fragilidad.
Métodos: Estudio descriptivo transversal en población mayor que vive en la comunidad. Para la muestra se seleccionaron a mayores de 64 años que respondieron a la ola ocho de la encuesta SHARE. La variable de resultado principal fue la fragilidad medida por el fenotipo Fried operacionalizado a SHARE-FI. Las variables independientes fueron los DSS. Se realizó un análisis bivariante a través de pruebas de contraste de hipótesis no paramétricas e intervalos de confianza 95%. Resultados: Se midió la fragilidad en 1470 personas. La prevalencia global de fragilidad fue de 10,61% (IC 95% 9-12,2), siendo del 15% en mujeres, tres veces y medio mayor que en hombres y del 22,6% en mayores de 80 años. Algunas de las variables que se relacionaron con fragilidad fueron vivir solo (OR 2,38; IC 95% 1,89-3,01), limitaciones ABVD (OR 11,09; IC 95% 7,59-16,18), soledad (OR 3,01; IC 95% 2,39-3,79). Otras variables como no tener depresión (OR 0,18; IC 95% 0,14-0,23) y tener alta calidad de vida (OR 0,18; IC95% 0,14-0,23) fueron factores de protección frente a fragilidad.
Conclusiones: Uno de cada diez mayores de 64 años es frágil, existen diferencias en género y edad, siendo las mujeres y los mayores de 80 años los grupos con mayor fragilidad. Los DSS influyeron en la variabilidad de la fragilidad en población mayor.
