Amnioinfusion has been historically used to improve neonatal outcomes in cases of meconium aspiration during labor. We present the case of a full-term, low-risk pregnant woman with meconium-stained amniotic fluid in active labor, in which its use was ruled out due to the presence of a cardiotocographic record with atypical variable decelerations. Although some studies suggest that amnioinfusion may reduce the risk of meconium aspiration, these studies have biases and limitations. Its use is recommended in centers with limited resources for maternal-fetal monitoring and in cases of suspected funicular compression. However, in cardiotocographic records with atypical variables, other measures should be considered to address possible placental problems and improve the fetal situation.
