ICU delirium, a narrative review of the existing evidence

Introduction. Delirium is the most common manifestation of acute brain dysfunction in critically ill patients. With an incidence of 40 to 80%, its identification, prevention and treatment are increasingly considered as priorities in public health.
Objectives. Increase nursing knowledge about the management of delirium in critically ill adults in intensive care units.
Methodology. A narrative review has been developed in the PubMed, Cochrane, CINHAL and ClinicalKey databases.
Results. It includes 18 systematic reviews and meta-analyzes, 11 integrative and / or narrative reviews, 5 randomized clinical trials, 5 clinical practice guidelines or parts thereof, and 10 studies that include cohort studies, validation studies, observational studies, and multicenter studies. .
Discussion. More and higher quality research is required to elucidate certain aspects of delirium. It is recommended to follow current guidelines for the management of delirium and implement them in all ICUs.
Conclusions. Delirium is consistently associated with adverse short-term and long-term outcomes. Non-pharmacological preventive measures and the detection of patients at risk are recommended. It is recommended to regularly assess delirium, and the application of etiological and non-pharmacological treatment. A change is needed in ICU work dynamics that allows progress in improving delirium management, and this change should be led by nursing.
